We are looking to appoint enthusiastic and committed individuals to deliver a residential short breaks service to disabled children and young people. The successful applicants will display a creative, patient and caring but dynamic approach to young people and must be able to create and follow care plans.You will have experience of, and a genuine passion for, working with learning disabled young people aged 0 to 17 who may display complex challenging behaviours and may have a range of physical disabilities and clinical needs. Experience and knowledge of the impact of, and approaches to working with young people on the autistic spectrum, along with experience of working with young people who have complex needs such as sensory/communication difficulties and clinical needs is important.
You will be able to communicate effectively with young people and can build positive relationships with young people and their families, to enable young people’s care needs to be positively met.
You will understand the physical, emotional, and social needs of young people and their development and the role of short break care within this. You will be fun and child focussed and able to work as part of a team to plan and deliver care.
You must have excellent ability in written and numerical skills. Health and Social Care CYP NVQ 3 / Level 3 Diploma are desirable or the commitment to study to achieve this is essential.
Due to the nature of the post, applicants must be 22 years of age or above.
Shift work is an essential part of the post and includes evening and weekend working plus occasional sleep-in duties when required and waking night care in emergencies. You must be flexible and be able to change shifts to accommodate the needs of the service.

The Role:
There are 2 exciting social work opportunities that have arisen within Derby City Council’s family and friends team. We are looking for experienced social workers who have the ambition to join us at this exciting moment, to be part of the Council’s journey and commitment to keeping children within their family networks when it is no longer possible to live with their birth parents. To achieve this, we are looking for highly skilled experienced social workers who have exceptional assessment skills, to come and join us to ensure that our children receive the highest quality family-based care within extended family. As well as completing detailed and holistic assessments, part of this role will also include supporting Friends and Family carers post order, developing robust support plans, as well as providing support and training individually to carers as well as the wider Friends and Family community of carers.
In addition to the above to meet the recommendations of the Kinship Care strategy and Kinship Care statutory guidance 2024 the part time position (3 days) role will have a focus on working with families post order’s being made, when a SGO or CAO has been granted. Part of this role will be supporting and assessing where there is risk of breakdown (including the need to consider if a single assessment is required) alongside completing needs assessments and ongoing work with partner agencies for the adoption and special guardianship support fund applications for therapeutic service input for children and families.
About You:
We need practitioners who is committed to children achieving good outcomes whilst living with their extended family or friends with an energetic can-do/will do approach then this may be the role for you! You will need to have had experience of working with children and families during and after care proceedings, PLO processes and completing some experiences of fostering/SGO assessments. Strong assessment, analytical and written skills are also required with an eye for creative thinking, strengths base practice and finding achievable solutions. You will need to produce a range of complex, robust, court-ready assessments, in the form of reg 24 fostering assessments, SGO and private fostering assessments, often within tight court timescales. You should feel confident in presenting these reports to court, to fostering panel and SGO panel. You will have a high level of interpersonal relationship-based skills, with an ability to work effectively with colleagues, partners, children, and carers, whilst providing appropriate measures to ensure that the child always remains at the centre of our planning and practice. You have a demonstrable understanding of key legislation, which equips you to analyse and intervene effectively in situations that are increasingly complex and challenging.

An exciting transformation project “Care Homes Falls Support Service” is being developed between Derby City Council Carelink and Team Up Derbyshire working together in partnership.
The service will be working with care homes to support them with management of falls and to improve the experience and outcomes for residents who have fallen.
We are looking to recruit three support workers who will promote falls prevention and provide care homes with falls lifting equipment along with the appropriate training to use the equipment and support them with post fall management. This will involve working alongside care home staff to effectively manage residents who are at risk of falling and linking in with the aligned PCN and MDT to ensure that local pathways of care are embedded, ensuring community based clinical advice and guidance is available when required.
The Team will work within the hours of 8am – 6pm Monday to Friday. However, they will be expected to occasionally work outside of these hours, including weekends, to meet the demands of the work and to fit in with care home routines.
