Mobile Care Support Worker
Job description
Directorate: People Services
Hours: 25 hours. (additional hours are available but not guaranteed)
Contract type: Permanent
Location: Mobile Across Derby City
25 hours weekly average, working 4 days on/4 days off on a rolling rota, working mornings for 2 working weeks and evenings for 1 working week at the following shift times (with thirty minute unpaid
break each shift) :
• Mornings: 7.15am to 2.45pm
• Afternoons: 2.30am to 10.00pm
Social care has been prominent in the media during recent months and has highlighted the amazing role that care support staff play in our community.
As a result of increasing demands for our service, an exciting opportunity has arisen for Mobile Care Support Staff to join our award- winning Home First team.
Working closely with our NHS colleagues, we are responsive to the needs of our customers by supporting the prevention of hospital admission and facilitating hospital discharge by providing short-term assessment support to people in their own homes and within our integrated Assessment Hubs at Perth House and Arboretum House.
We look after some of the most vulnerable and frail people in our community who have been discharged from hospital, or for those who experience a short period of ill health enabling them to stay at home rather than be in a hospital bed. We assess customer’s needs and work with them to be as independent as possible.
Working as part of our valued and vibrant care support team you'll be ensuring our customers are supported in assisting them with everyday tasks such as washing and dressing, meal preparation and medication. Working in this fast-paced team you'll work closely with social care practitioners and health care professionals.
