Derby City Council and Southern Derbyshire Clinical Commissioning Group are proud to be seen as one of the pioneers in developing the Local Area Coordination approach in England with our Local Area Coordination team sitting at the heart of our new plans to deliver joined up care.
An exciting opportunity has arisen to join this established team as a direct result of Derby's successful bid to the Department of Education's Innovation Fund. This will see the team increase its remit to begin working with young people leaving care.
Local Area Coordination is an exciting approach to building inclusive, supportive communities that help people stay strong and resilient, and which results in services which are more personal, local, flexible and accountable. We are looking for exceptional individuals who combine a passion for their local community with a commitment to working alongside people in very practical ways - always looking for local solutions that will enhance the persons participation in, and contribution to, their local community.
* You will need experience of working alongside individuals with disabilities, mental health problems, older people, families and/or carers and a background or interest in community capacity building. * You will have an understanding and practical experience of advocacy and supporting people to plan for the future. * You will be a creative thinker who is able to work in innovative ways and you must be committed to the principles of co-production. * You will be educated to degree level or equivalent, or have experience of working in a similar role at a similar level.

It will be an opportunity to speak to members of the Local Area Coordination team, ask questions and find out more about the role.
