We are proud of our award- winning Home First team whose short-term support enables customers to maintain or regain their independence to live in their own homes, or whilst having assessment in our integrated Assessment Hub at Perth House, Derby.
We now
have opportunities for a full-time 37 hour or 25 hour part time position of Admission and Discharge Customer Coordinator to work within our dedicated team.
Working collaboratively and closely with our NHS colleagues and multi- disciplinary partners, we are responsive to the needs of our customers by supporting the prevention of hospital admission and facilitating hospital discharge. The Admission and Discharge coordinators are responsible for the safe, effective, timely customer admissions and discharges to and from all our Pathway 1 Community and Pathway 2 Assessment Bed Hub services, ensuring all correct documentation is in place.
Based at our Assessment Hub Perth House, Derby you will be responsible for the capacity and bed allocation checking within our services, liaising with our Hospital to Home and Programming team colleagues, ensuring the accurate recording of information on relevant systems. Central to this role you will coordinate and convey the appropriate responses and actions to queries in the day-to-day communications with our customers, families, staff teams and our partners.
You will support the Registered Managers to ensure the safety of the Assessment centre building, hygiene and infection control practices are carried out, including the checking of risk assessments completed by Senior Care Leaders.
Candidates must be flexible, able to work in a fast-paced environment and willing to work the following shift times on a ‘weekly’ 4 days on/4 days off rolling rota basis:
25 hour position:
7.15am to 2.45pm (with a 30-minute unpaid break)
2.30pm to 10.15pm (with a 30-minute unpaid break)
37 hour position:
7.15am to 6.15pm (with 30-minute unpaid break)
11.15am to 10.15pm (with 30 minute unpaid break)
You must be able to demonstrate a calm, positive approach with a keen willingness to learn, be self-motivated and enjoy working in a rapid response setting. Ideally you will have experience in the care field or transferrable skills and looking for that supervisory role.
