We are looking for Support Workers to join our award winning team. Our team of Support Workers work closely with our NHS colleagues in our short term, rapid response service being responsive to the needs of our Derby customers by supporting the prevention of hospital admission and facilitating hospital discharge. Home First Support Workers work to maximise customers independence, or help regain their independence, to enable customers to live in the place they call home. As a Support Worker you will be supporting our customers during their short term stay in our integrated Assessment Hub at Perth House, Derby prior to them returning home and on occasion supporting your colleagues in customers own homes across the City of Derby.
