HOME FIRST
This is an exciting opportunity to work in a new role within the Home First team at our Assessment Hub Perth House, visiting customers in their own homes and being responsive in identifying the ongoing assessment needs for our customers following a short term Home First service which has prevented a customer from being admitted to hospital or in supporting their discharge home.
This post will also be involved in supporting our development integration with DCHS. (Derbyshire Community Health Services) and our colleagues at the Royal Derby Hospital.
The postholder will be a positive, 'can do' person who will work closely with our Senior Support Leaders and Programmer Co-ordinators in the rapid, timely assessment and transfer of services from the Home First Team.
You will need to be able to work under your own initiative in this fast paced environment and have some experience of Assessing, Monitoring and Reviewing customers care needs, demonstrate good IT and have good organisational and communication skills.
The post holder must have a full UK driving licence, use of car for work purposes together with business use insurance. A mileage rate is payable.
