Home First team are looking for Mobile Senior Support Leaders to join our award winning team. We work closely with NHS colleagues, being responsive to the needs of our customers by supporting the prevention of hospital admission and facilitating hospital discharge. The Senior Leader will be working in the Community in customers own homes with our Support worker teams, but you may be asked to work in our integrated Assessment Bed Hub at Perth House when you first start with us and from time to time during your working weeks. We are looking for Senior Support Leaders who have the knowledge and skills to demonstrate leadership to our Support Worker staff team and able to assess customer’s needs and direct our Support Workers to work with our customers to be as independent as possible by providing short term assessment support. An important part of our work is looking after customers, across the city of Derby, including those who have been referred to our delirium pathway - helping everyone to remain at home in familiar surroundings rather than be in a hospital bed.

The OpportunityIn the role of Mobile Senior Support Leader, here’s an example of what you’ll be doing to make an impact:• Helping build customers confidence in their day-to-day activity.• Showing customers how to do things differently yet achieve the same goal.• Supporting and giving direction to our Support Workers• Assessing and supporting with personal care• Assessing and supporting with dressing/undressing• Assessing and supporting with meal preparation• Dealing with medication queries and competenciesWe can capture the tasks that you will undertake during the customers short time with us, but you need to be a special person to meet the emotions and understand people, lend an ear to listen, calm a situation, empathise or just simply taking the time to talk to someone and be part of their day. Working as part of our valued and vibrant care support team you'll be supported by our Management Team and work closely with Social Care Practitioners and Health professionals.

• Please note: This is a mobile post, therefore you must have use of a car/motorcycle available for business use and hold a full UK driving licence.• Have NVQ 2 in Care or equivalent with a desire to complete NVQ level 3 in Health and Social Care.• Good working knowledge of risk assessment • Able to record on IT systems.• Ability to demonstrate your understanding and knowledge to others.• A calm, organised approach.• Are self- motivated with a desire to make a difference.• A 'can do attitude' and open mind.• Caring nature. • Reliable and flexible.• Willingness to learn.• Good level of written and spoken English. • A right to work in the UK.
